OASIS Members Approve Emergency Data Exchange Standards for Resource Messaging and Hospital Availability

Boston, MA, USA; 8 January 2009 — OASIS, the international open standards consortium, today announced that its members have approved the Emergency Data Exchange Language Resource Messaging (EDXL-RM) 1.0 and the EDXL Hospital AVailability Exchange (HAVE) 1.0 specifications as OASIS Standards, a status that signifies the highest level of ratification. EDXL-RM describes a suite of standard messages for sharing data among information systems that coordinate requests for emergency equipment, supplies, and people. HAVE specifies an XML document format that allows a hospital's status, services, and resources (including bed capacity, emergency department status, and available service coverage) to be communicated. Both standards were developed through an open process by the OASIS Emergency Management Technical Committee.

"Both EDXL-HAVE and EDXL-RM represent significant achievements in messaging standards development," said FEMA DM Program Manager Sarah Hyder. "Implementation of these standards not only enhances emergency management, but also improves the speed and quality of response activities. Once in the hands of emergency responders and managers, data messaging standards such as HAVE and RM will tremendously improve the exchange of information in real time."

"Specific, standardized messages are critical for coordinating emergency response--particularly when more than one profession or governmental jurisdiction is involved," noted Elysa Jones of Warning Systems Inc (WSI), chair of the OASIS Emergency Management Technical Committee. "EDXL-RM and HAVE make it possible for critical information about life saving resources to be shared across local, state, tribal, national and non-governmental organizations to support a coordinated response to an emergency."

"We are committed to working with standards development organizations, including OASIS, to integrate EDXL data messaging standards into the resources that emergency responders rely upon to keep the Nation safe."

EDXL-RM specifies a document format that allows the communication of information about resources, such as requests for obtaining resources, responses to these requests by potential suppliers, and information on the status and location of resources.

HAVE allows emergency dispatchers and managers to make sound logistics decisions on where to route victims based on accurate hospital bed availability, status, services, and capacity data. Although some hospitals currently use proprietary technology to publish this kind of information, only those parties with the same systems can access their data.

"Isolated pockets of communication are simply not sufficient for large-scale emergencies," said Laurent Liscia, executive director of OASIS. "By presenting data in a standardized format, HAVE removes this barrier, enabling all parties involved in emergency response (other hospitals, EOCs, 9-1-1 centers, EMS responders, etc.) to view the information they need."

In accordance with eligibility requirements for all OASIS Standards, successful use of EDXL-RM was verified by NICTA, NuParadigm, and OSS Nokalva; HAVE was verified by ESI, Evolution Technologies, and NuParadigm. Both standards were developed under the Royalty-Free on RAND mode of the OASIS Intellectual Property Rights Policy.

Participation in the Emergency Management Technical Committee remains open to all companies, non-profit groups, government agencies, academic institutions, and individuals. Archives of the work are publicly accessible, and OASIS offers a mechanism for public comment.

Consumer empowerment improves quality of care

NAPLES, FL – December 18, 2008 – HIPAAT Inc. (HIPAAT), the leading provider of consent management solutions to the healthcare industry, partnered with IBM in this week’s successful North Carolina Healthcare Information and Communications Alliance, Inc. (NCHICA) and MedVirginia demonstrations of the Nationwide Health Information Network (NHIN) Trial Implementations. The Washington, DC presentations highlighted the importance of consumer empowerment and patient privacy.

HIPAAT’s SOA-based consent engine, Privacy eSuite, provides consent management services to enable authorized healthcare providers and patients to securely and privately exchange medical records. The consent engine was an integral component of IBM’s Health Information Service Provider (HSP) solution for the demonstration. Privacy eSuite allows patients to establish their own consent directives. It then determines if a given healthcare provider is authorized to access the patients’ personal health information (PHI), based on those directives. Once authorized, the physician has straightforward access to requested PHI through the secure nationwide network of health information.

The IBM HSP provided the supporting architecture and operations to power the cross-organizational services for two of the nine demonstrating NHIN Health Information Exchange (HIE) contractors: NCHICA and MedVirginia.

NCHICA and MedVirginia, under contract to the U.S. Department of Health and Human Services (HHS) and the Office of the National Coordinator for Health Information Technology (ONC), have spent the past fifteen months propelling the goal of a “network of networks.” The final deliverable to HHS/ONC was a live national demonstration of privacy-sensitive access control and information exchange across all nine HIEs through a series of seven use cases.

NCHICA demonstrated the Consumer Access to Clinical Information (CACI) use case. In the demo, “Susan Clark” was a prototypical patient with medical records spread among various private and governmental providers. Susan selected and populated a Personal Health Record (PHR) with her own PHI, which she imported through the NHIN from her physicians and other providers. She then chose what portion of her health records she wanted to share by role (e.g., physician, nurse, administrator), by named physician, or even by document type. Susan recorded her consent preferences in her PHR, which in turn stored the directives in Privacy eSuite through IBM’s HSP. Selected PHI was available to authorized users of the North Carolina HIE and the NHIN through the IBM HSP.

Aetna, Microsoft Integrate Online Health Tools

Aetna has made its PHR tool compatible with Microsoft HealthVault.

Under an agreement that could represent an early step towards more portable, carrier-agnostic PHRs (personal health records) for consumers, Hartford, Conn.-based Aetna announced in late October that it will provide members with the capability to transfer data contained with their carrier-provided PHRs to Microsoft's Web-based consumer health platform, HealthVault.

"We really see this as the first step," says Dan Greden, Aetna's head of eHealth product management. "This is just the beginning of using a members' personal health data to integrate with other applications and other tools. Down the road, we might even see a scenario where you can integrate it with tools that physicians are using directly."

The extent to which the data contained within PHRs is portable is a key factor when determining the long-term value of online tools like PHRs and similar platforms such as HealthVault. Interoperability will allow users to essentially take their data with them when they switch health insurers. And ultimately, the more these online tools from disparate sources can interoperate, the more complete an individual's health record can become.

Initially, the partnership will allow members to export data from Aetna's PHR into HealthVault. In 2009, users will be able to import data from HealthVault into an Aetna PHR. "We made a very deliberate decision for the first capability to be the ability to take data out of our record and [export] it somewhere else. For whatever reason they are leaving us and going somewhere else, [the member needs] to be able to take their data with them," says Greden.

To integrate with HealthVault, Aetna leveraged PHR portability standards from AHIP (America's Health Insurance Plans) that it helped develop along with several other carriers. Those standards, Greden says, align with the Quality of Care Document (CCD) XML standard contained within industry group Healthcare Level 7's (HL7) guidelines.

"The reason we chose to use the CCD standard is that, much further down the road, we think it will facilitate better interchange with other things that are already in the healthcare technology space," Greden explains. "We really see this as the first step. This is not the end of it."

When Microsoft launched HealthVault in October 2007, it announced several related partnerships with health care providers and other organizations, but no insurance carriers. It was a conspicuous omission, considering that claims data represents what many agree is the most complete and clinically accurate record of an individual's health currently available.

However, the Redmond, Wash.-based company did recognize the value that insurers could provide to HealthVault. "We are talking with many or most of the major [health] plans nationwide," Sean Nolan, chief architect Microsoft's health solutions group said at the time. "We really hope that they do become a key part of the [HealthVault] ecosystem because there is certainly data that consumers have within their plans, both financial and clinical, that would be extremely useful to them."

Greden confirmed that dialogue regarding an Aetna-Microsoft partnership began during the planning stages of HealthVault. "It was clear right away that we shared that view of the benefits of empowering the consumer with good information and good information technology," he says, adding that Microsoft described HealthVault as a platform, as opposed to a PHR. "They've built a platform and an environment. We've got the PHR, which is a valuable tool for members, a valuable healthcare application. We can work together in this context to further our shared goals."

In the fall of 2007 -- when Aetna and Microsoft took the first steps towards portability -- the carrier was only piloting its PHR tool, which it developed with an assist from ActiveHealth Management, a wholly-owned Aetna subsidiary that has another, unrelated partnership with HealthVault. In 2008, Aetna initiated a broad rollout of its PHR to members.

A majority of the 6 million Aetna members that have adopted the PHR have now had it for six to seven months. Adoption is still growing rapidly. "What's interesting is that, of the people that visited [their PHR], 40 percent have come back within a small timeframe. So, we're finding that when people do use it, they're finding it valuable and are coming back," Greden says. "With this capability with Microsoft, they can take their data with them to HealthVault and do more with it there."

Cleveland Clinic and Microsoft Collaborate in First Multi-Disease, Multi-Device Pilot in Clinical Setting

CLEVELAND, Nov 10, 2008 /PRNewswire via COMTEX/ -- Cleveland Clinic, a world-class academic medical institution and leader in health information technology, today announced collaboration with Microsoft Corp., a worldwide leader in software, to pilot the patient-controlled data exchange between Microsoft(R) HealthVault(TM), a Web-based personal health platform; a suite of widely-used digital medical devices; and eCleveland Clinic MyChart(R), Cleveland Clinic's electronic personal health record (PHR) system.

Today, more than 150,000 Cleveland Clinic patients use eCleveland Clinic MyChart(R). The short-term pilot, begun on Monday, Nov. 3, is the first pilot in the country to follow multiple diseases in the clinical delivery setting using multiple at-home devices such as glucometers, heart rate monitors, weight scales and blood pressure monitors.

The pilot will be a physician-driven, invitation-only opportunity offered to a group of Cleveland Clinic PHR users in the areas of diabetes, hypertension and heart failure. Cleveland Clinic plans to enroll approximately 400 patients and aims to demonstrate that the program will enable patients and physicians to better manage and track chronic diseases from home, using the patient's own computer.

"As the prevalence of chronic disease rises at an alarming rate in the United States, the strain on our nation's economy and healthcare system can not be relieved without the creation of innovative, cost-effective solutions that involve patients in proactively managing their healthcare," said C. Patients will be provided HealthVault-enabled digital devices, such as blood pressure monitors and glucometers, and asked to perform regular health monitoring. By connecting the device(s) to their home computers, their health information will be uploaded, with their consent, to a personal HealthVault account controlled by the patient, and then sent to their Cleveland Clinic MyChart account. This data will create an online log of the readings that will be available to the patient's physician. Data sent from a patient's HealthVault account to MyChart can not be changed or altered in any way.

Cleveland Clinic tests Microsoft health-monitoring technology geared for patients to use

The Cleveland Clinic is testing an electronic health technology that could help transform the way its doctors and other caregivers practice medicine.

The Clinic is partnering with Microsoft HealthVault to enable certain patients to monitor chronic conditions -- high blood pressure, diabetes and heart failure -- at home. These patients will use high-tech devices, home computers and the Internet to keep Clinic doctors posted on their conditions.

The doctors could rely on the information to adjust medications or order aggressive medical care without seeing patients for office visits. Early medical interventions could lead to healthier patients and fewer hospital admissions, lowering costs.

"There is evidence in the literature that these kinds of more continuous monitoring programs do improve the health of patients over time," said Dr. C. Martin Harris, the Clinic's chief information officer.

"This allows us to begin thinking about treating patients when they're not in a hospital bed or in our office," Harris said.

The Clinic and Microsoft are providing patients with digital blood pressure and heart rate monitors, as well as blood sugar meters, said Peter Neupert, vice president of Microsoft's Health Solutions Group.

The monitors and meters can be plugged into computers, which upload information into HealthVault -- the online service that enables users to collect, organize and share their health information.

The Clinic's electronic health record system, called MyChart, would grab the information from HealthVault, making it available to doctors in the form they choose, Neupert said.

The Clinic hopes to involve between 400 and 500 patients in the 90-day pilot project, which began Nov. 5, Harris said.

Frequent monitoring of chronic conditions could arm patients with information to better manage their health.

"One of the good parts of MyChart is it runs an ongoing graph of my blood pressure readings," said William Joseph Moore of Lorain, who has participated in the HealthVault pilot for about a week.

Moore takes his blood pressure reading at least once a day. "It gives you a good opportunity to interact with your doctor on a daily basis," he said.

Because home monitoring technologies have the potential for lowering the cost of health care, Moore also believes they could have "far-reaching implications" for people who lack insurance or money to pay for regular doctor office visits.

Aetna to offer records in Microsoft HealthVault

INDIANAPOLIS (AP) — Health insurer Aetna Inc. is becoming the first health insurer to team with Microsoft Corp. to give its customers an Internet-based vault for storing medical records they can access even if they change jobs or leave their health plan.

Starting next month, Hartford, Conn.-based Aetna will allow some customers to transfer electronic personal health records to Microsoft's HealthVault, a platform that lets care providers look at the information, if they have patient permission.

The vault will give the insurer's customers "continuous access" to their claims information and anything the patient wants to add, like clinical data or past medical records, Aetna President Mark Bertolini said.

The insurer sees several advantages in the move, including improved communication with providers.

"We can avoid duplicate testing, we can avoid mistakes that occur as a result of not understanding the member's complete condition," he said. "Because we don't have a national health information technology network, this will stand as a first-generation of that kind of capability."

The announcement marks the latest step in a push by several companies into the storing and sharing of medical records.

Google Inc. opened a Web site earlier this year that allows people to store health information such as doctor, hospital and pharmacy records in one place. IBM and Duke University also have created a Web site that allows people to pay medical bills, schedule doctor appointments and store medical records, among other things.

Redmond, Wash.-based Microsoft launched the free HealthVault Web site a year ago. About 40 companies currently allow customers to store information on it, a list that includes hospitals and CVS Caremark's Minute Clinic. Aetna would be the first health benefits company to do so, according to Microsoft.

The customer has complete control over who looks at the data and what information they see, said Peter Neupert, vice president for Microsoft's Health Solutions Group. He said doctors and providers, even Microsoft officials, will not be able to access the information unless they receive permission first from the patient.

"This is simply a place for you, like a safe deposit box, to store your data that is yours and yours alone to decide how you want to use it," he said, noting that it won't be used for data mining.

The information also will be protected under the new "Connecting for Health" guidelines devised earlier this year by Microsoft, Google and others that wanted to set ground rules for keeping sensitive information private.

Experts have said that consumers are wary of the protection offered for electronic medical records, but Neupert said Microsoft is "very confident and vigilant about providing security for this data."

Aetna makes electronic personal health records available to about 6 million customers, and the fee for that is part of their premium. But the insurer plans to charge nothing for the HealthVault transfer.

The insurer expects to recover costs tied to setting up the agreement.

"Right now, we spend tens of millions of dollars answering the phone telling providers if a person is eligible or what their benefits are," Bertolini said. "With the availability of this information, those calls go away."

The primary benefit is a marked reduction in paperwork common to cardiac rehabilitation programs. Q-Tel also makes data transfer to hospital information systems (HIS) and electronic medical record (EMR) systems easy. Patient Intake Assessment – Documents the assessment of every patient in the clinical, behavioral, health, and services domains, and captures information for all four domains on one form.
Patient Query Report – Can perform custom queries of the patient database to help identify attendance counts, referring physicians, or patients who match other specific criteria. A simple point-and-click interface reveals the answers to these and countless other queries.
Customized Patient Outcomes Graphs – Creates individualized graphs of the outcomes parameters needed for each patient. The most relevant parameters are charted – and can be altered in real-time – to show the most relevant information for each patient.

Q-Tel is highly configurable for additional dedicated workstations or for extra monitoring stations as class sizes increase. The Pocket Assistant allows the rehabilitation team to input session parameters from anywhere on the floor. This allows more personal interaction with patients.
